REGISTRATION

Register early & save!

To register in advance, simply print our registration form; complete all sections of the form including the selection of either morning, afternoon or all-day sessions; enclose your payment and send! Please use one form per person. We’ll begin accepting registrations on August 8, 2002.

Advance registration forms must be received by October 4, 2002 to receive the discounted rate. Unfortunately, we cannot accept electronic submissions. To ensure your application is received by the advance registration deadline, please mail a minimum of five days in advance.

Registration Fees  

Morning registration fee includes conference handouts, continental breakfast, lunch and all sessions through 1:45 PM. Afternoon registration fee includes conference handouts, lunch, afternoon snack and all sessions after 12:45 PM. All-day registration fee includes conference handouts, continental breakfast, lunch, afternoon snack and all sessions through 4:00 PM. Early registration is recommended, as seating is limited.

Please include full payment with your registration. Make checks payable to The Family & Workplace Connection or charge your VISA or MasterCard. Please forward registration and payment to:

Substitutions and Cancellations

Substitutions are permitted at any time for confirmed conference registrants. Written cancellation requests must be received by October 23, 2002. Cancellation requests received after that date will be processed less a $30 fee. No phone cancellations, please. No refunds after October 30, 2002.

Special Needs

If you have a special physical need that may impact your ability to attend our conference or if you have a special dietetic need, please check the appropriate box on your registration form. A member of our registration team will contact you to see how we may be able to provide assistance.
